Menominee / Marinette Youth Fundraiser 5 on 5 Pot Luck Tourney
When:
The 1 day potluck tournament will be on Saturday December 2, 2023, 11am-7pm. Registration is at 9:30am and the Arrival/Team Meeting is at 10:15am Poolside, full attendance is Mandatory.
There will be a Rookie & Intermediate Clinic on Sunday December 3, from 8am-12noon. There is no cost for this as it’s included with the entry fee. Please select your attendance on the registration form.
Format:
Potluck Teams – Random Teams will be comprised from all players registered with a round robin format.
There will also be a Youth and Rookie Game. Youth is under 18, and Rookie have been playing less than 1 year. Please sign up on registration form

The pool is a 25 yards x 6 lanes, 3.5 – 6.5 feet deep with short slope and great visibility. The floor is smooth and fast 1″ tiles.

Player Fees:
Adults: $70.00 per player which includes: Tournament fees, Tournament T-Shirt, Food & Drink at the pool and an all you can eat buffet dinner after Saturday’s play.
All Students: $40.00 per player which includes: Tournament fees, Tournament T-Shirt, Food & Drink at the pool and an all you can eat buffet dinner after Saturday’s play.

Please only one spreadsheet for each team location,( ie. Minnesota or Chicago) individuals without a club may fill out their own form.
Step 2: Fill out sheet electronically fully and email electronic copy back to Steve. All entries must be received before November 20th, 2023 to avoid a $10.00 per player late fee penalty. Print off Roster Spreadsheet, Mail Spreadsheet along with entry fee cheque to:

Step 4: Menominee Marinette Underwater Hockey Waivers must be signed (by all participants) and submitted before the tournament begins, best to scan and email them to me with registration form. To download the waiver, use this link: MM Waiver
Step 5: No Waivers – No Play – No Exceptions!
Time Line:
November 20th, 2023 – Deadline for entry fee, mailed checks must be postmarked by this date ($10.00 per player penalty if paid after November 20th, 2023), or paypal as a group.
November 27th, 2023 – Final deadline to participate, no players/teams will be admitted after this date. Finalized rosters must be submitted electronically. Add $10.00 per player late penalty to registration.
No Refunds after November 27th, 2023.

Cost: Group Rate is $79.00 plus tax for a 2 double bed room. The cut off for the block reservation rate is November 20, 2023.

Banquet/Awards/Raffle:
Following the Pot Luck tournament at 7-7:30pm, there will be an all you can eat buffet dinner, with pop/water/coffee/tea (included in the tournament cost) and the Pot Luck Team awards will be presented at this time along with the raffle drawings.
It will be the King’s Buffet located at:

They have a side room/area in which our group will eat. Dinner starts at 7:30pm and closes at 9:30pm.
After Party: Three Bridge Brewery – Brewery Event (9pm-11pm)
We have teamed up with Three Bridge Brewery in Menominee, Michigan for a Tap Room Event. The cost is $15.00 and includes your first pint of beer. All beers after the first pint are at your own cost. Please select on the registration form your intent to attend this event along with your $15.00 payment at the time of registrations due. Venue is about 10 minutes from the tournament hotel. You must be 21 to attend.
